What to expect from the Green Card Immigration medical Exam
- Comprehensive I-693 Form medical evaluation and physical examination
- All laboratory work including required gonorrhea, syphilis, and tuberculosis test
- Accurate medical examination that follows the Technical Instructions established by the CDC (Center for Disease Control & Prevention) and required by USCIS
- All required vaccinations
- If all medical work up is negative you can expect your sealed I-693 Form in about 2- 3 days to provide to USCIS (copies also given to applicant)
How to prepare for your Immigration medical
- Bring a valid government issued photo I.D. (i.e valid unexpired driver's license, passport or work authorization card)
- If an applicant is 14 y.o. or younger identification that shows name and place of birth and parent's full name such as birth certificate (please translate any foreign birth certificates to English) or bring an affidavit
- Vaccination and immunization record. Bring Vaccination and immunization record with you or fax to 718-701-5949 or email to doctor@civilsurgeon.net
- If necessary please bring a friend that speaks your language as well as English
